It expected conformity of real cost and the new INA-CBG's package tariff after using JKN, especially in cost expensive disease like acute myocardial infarction (AMI). This study the aimed to know conformity of real cost and INA-CBG's package tariff after JKN, to observe difference therapy and outcome clinic of AMI. This was an analitical-descriptively research with of cross sectional research design. The data found from medical record and departemen of financial in patient Jamkesmas and JKN during hospitalization on September 2013-April 2014. The cost was observe such as direct medical cost and direct non medical cost during hospitalization. The clinical outcome evaluated of pain duration and lenght of stay (LOS). The result of this study showed the varians of choice therapy of trombolisist anticoagulant, beta blocker, ARB, and ACEI theraphy after JKN. The three most significant cost component included proceeding, drug or medical materials and acomodation. Difference real cost and the INA-CBG's tariff after JKN in AMI without intervension is -Rp. 8.965.305 at the severity Ievel I and + Rp.6.981.117 at the severity level II. AMI with intervention cath difference real cost and the INA-CBG's tariff is -Rp. 8.965.305 at the severity level I and -Rp.6.695.372 at the severity level II. AMI with percutan coronery intervention (PCI) difference cost -Rp.144.579.485, \-Rp.7.738.792, +Rp.21.743.056 at the severety level I,II, and III. Before application JKN, LOS in patients with AMI without intervention is 6,25 days and 9 days at the severety level I and III. After JKN LOS to be 5,3 days and 8 days.
