| Abstrak | : | Hipertension is often called ?the silent killer?, because no symptoms. Based on Riset Kesehatan Dasar (Riskesda) 2013 in Indonesia prevalension of hipertension reached 25,8%, while in Kecamatan Prambanan hypertension is one of ten most disease. Hypertension take a long time in terapeutic process, so the cost required is greater. Demands of people who want to get the treatment of hypertension as effectively as possible with the lowest possible cost encourage this research, the aim is to provide an overview of antihipertension class that has more cost effective from the use of ACE inhibitors, ?-blockers, diuretics, or calcium channel blockers. The method of the research is retrospective cohort study. Calculated costs are direct medical costs by hospital perspective, and the effectiveness of therapy seen in the proportion of patients achieving the target. Cost-effectiveness analysis performed by calculating the ACER (Average Cost Effectiveness Ratio) and ICER (Incremental Cost Effectiveness Ratio). The results of the study from 65 patients, most widely used antihypertensive classes are diuretics (31.32%). The most effective antihypertensive of non- assurance patient groups is ACEI group (100%), whereas patients of group assurance are diuretics (100%) and ACEI (100%). Antihypertensive most cost effective of non- assurance group are diuretics, the ACER value is 335.82, while based on the value of the ICER, the additional cost incurred to increase the effectiveness of HCT 1% if therapy is replaced with nifedipine, amounting Rp.1.513,6, while if therapy is replaced with captopril, amounting Rp.1.408,03. In assurance group, the most cost effective are diuretics, ICER values can not be calculated because of the effectiveness of furosemide has a maximum value (100%), so there is no increase in effectiveness can be calculated.
